Your completion of the PSS New Patient Questionnaire and Interview
PSS evaluation of goals and objectives
PSS assembly of Survey Envelopes
PSS express shipment of Survey Envelopes to you
Your assignment of names (New Patients) to the Survey Envelopes over the following
  four months (Note: Your fee to PSS includes all postage requirement. You do not have to pay for the postage to mail the Survey Envelopes. PSS applies stamps/postage on the Survey Envelopes and the return envelopes!)

PSS receives and evaluates the surveys
PSS sends, to you, a detailed report at the end of the survey period. This report covers
  the responses and comments received.
